• All Check Request forms are online via JotForm.  We will NOT be able to accept the old forms after August 1st.  Please use this new method as it is easier, faster, and more convenient for everyone!

TEXAS HOTEL STATE TAX EXEMPTION FORM

Please use this from to be exempt from paying STATE TAX at a Texas hotel.  Per Texas State law, Educational Entities are exempt from state hotel tax, but not local hotel tax. This exemption category includes in-state and out-of-state school districts, private or public elementary, middle and high schools, Texas Regional Education Service Centers and Texas institutions of higher education. A letter of tax exemption from the Comptroller of Public Accounts as an educational organization is not required, but an educational organization might have one. Out-of-state colleges and universities are not exempt.

COSTCO SALES TAX EXEMPTION CLAIM FORM

Please use this form to be reimbursed FROM COSTCO if you had to pay sales tax upon check out.  After completing the form (you will fill out the highlighted portions), email it to lhwildcatclub@gmail.com for the necessary signature.  Once it is signed electronically, you will receive a copy from The Wildcat Club. It is then UP TO YOU to submit the (1)form, (2) our Tax Exemption Certificate, AND (3) your receipt to apply for the exemption with Costco.  The Wildcat Club doesn't guarantee that Costco will accept your exemption claim.

RANK 1 Sports Forms

Richardson ISD and UIL require a parent/legal guardian of each student-athlete to read and sign off on the following sports forms every year. Student-athletes will not be allowed to begin participating in any tryout or practice, before, during, or after school, (both in-season and out of season) or games/matches until all forms are completed.

RISD requires that all student-athletes in grades 7-12 have a pre-participation physical exam, performed by a medical doctor, on file with the district annually in order to try out, practice or play.

Note that you will need your child’s RISD student ID number before you begin. Sports forms are available here and include the following:
