Please remember to NOT include sales tax in your check requests. Even if you had to pay it and it is on your receipt, please submit the check request amount WITHOUT sales tax. The Wildcat Club is a 501c3 and cannot reimburse for sales tax. The only exception to this rule, is if you are going to RESELL the items you purchased. Please reach out to the Treasurer lhwctreasurer@gmail.com if you have questions prior to filling out your check request. 

Completed check requests can be emailed to lhwildcatclub@gmail.com and The Wildcat Club will send out the request electronically for signatures.  You do NOT have to obtain signatures prior to email your form to lhwildcatclub@gmail.com.

COSTCO SALES TAX EXEMPTION CLAIM FORM

Please use this form to be reimbursed FROM COSTCO if you had to pay sales tax upon check out. After completing the form (you will fill out the highlighted portions), email it to lhwildcatclub@gmail.com for the necessary signature. Once it is signed electronically, you will receive a copy from The Wildcat Club. It is then UP TO YOU to submit the (1) form, (2) our Tax Exemption Certificate, AND (3) your receipt to apply for the exemption with Costco. The Wildcat Club doesn't guarantee that Costco will accept your exemption claim.
